Antidepressants as analgesics: an introduction.
نویسنده
چکیده
Tricyclic and certain other antidepressants are widely used to treat chronic and neuropathic pain states. The first report of such activity appeared 40 years ago, soon after the introduction of tricyclic antidepressants into clinical medicine. Their efficacy in this regard is now well established through both clinical trials, as well as systematic and meta-analysis approaches. Although the mechanistic basis of antidepressant action has been actively investigated since their introduction as psychotropic agents, a particular focus on mechanisms involved in analgesia is more recent. The mechanisms involved appear to differ — analgesic properties occur independently of antidepressant actions, with a faster onset of action, in lower doses, and with a differing profile of effective agents. Despite these distinctions, the relationship between affective state and the pain experience is well recognized, and this component of activity needs to be considered as well. Initial studies on the mechanisms of analgesia examined activity in traditional nociceptive tests in which antidepressants exhibit variable actions. With the introduction of tests specific for nerve injury states in the past decade, mechanisms specifically relevant to chronic pain and neuropathic conditions have received particular attention. The following articles arise from a symposium held at a meeting of the Canadian College of Neuropsychopharmacology in Halifax, NS, in June 1999. They review the analgesic effects of tricyclic and other classes of antidepressants in clinical studies (see Lynch), present recent insights into the mechanism by which antidepressants produce relief of depression, as well as a specific hypothesis of analgesic action (see Blier and Abbott), and consider the multiple mechanisms by which antidepressants produce analgesia, both centrally and peripherally (see Sawynok et al.). The articles are not meant to be exhaustive reviews of these aspects of antidepressant actions; the reader is referred to recent reviews for further considerations of these dimensions of action.

Antidepressants as analgesics: a review of randomized controlled trials.
This review provides an overview of 59 randomized placebo-controlled trials that examined the analgesic effect of antidepressants. To summarize, there is significant evidence that the tricyclic group of antidepressants is analgesic and that trazodone is not; the data regarding selective serotonin reuptake inhibitors are conflicting. To date, there are no randomized controlled trials examining t...
متن کاملTopical analgesics.
BACKGROUND AND OBJECTIVES Pain treatment involves the usage of common and opioid analgesics, nonsteroidal anti-inflammatory drugs (NSAIDs) and adjuvant analgesics. Traditionally, these drugs are administered systemically or into the neuraxis. However, when analgesics are applied through these pathways, they are associated with significant side effects, which can hinder its use. Topical administ...
متن کاملAntidepressants as analgesics: an overview of central and peripheral mechanisms of action.
Antidepressants, given systemically, are widely used for the treatment of various chronic and neuropathic pain conditions in humans. In animal studies, antidepressants exhibit analgesic properties in nociceptive, inflammatory and neuropathic test systems, with outcomes depending on the specific agent, the particular test, the route of administration and the treatment method used. Although early...
متن کاملSelf-poisonings with antidepressants and other psychotropics in an urban area of Sweden.
As part of a WHO project on parasuicide, the medications used for self-poisoning in the Stockholm area were studied. The prescribing rates of the medications were estimated from an independent survey of prescriptions. Anxiolytics, hypnotics, and analgesics were the drugs most commonly used for parasuicide. Related to prescribing rates, antipsychotics and anxiolytics represented an increased ris...
متن کاملNeuronal and immunological basis of action of antidepressants in chronic pain - clinical and experimental studies.
The current knowledge of the pharmacological actions of the tricyclic antidepressants (TCAs) has slowly evolved through their over 40-year history. Chronic pain represents one of the most important public health problems, and antidepressants are an essential part of the therapeutic strategy in addition to classical analgesics. This article reviews the available evidence on the efficacy and safe...
